Concerned, Inc. provides a variety of services to help individuals in our community. These services are designed to support individuals with disabilities. Our function in providing these services is to assist all person(s) served in becoming as independent as possible in all areas of their lives.
Concerned, Inc. is continually assessing the needs of our community. We will provide new services as the needs for those services become apparent. As these services are developed, they will be added to our list of services included in our web site.

SCL Supported Community Living | IEP Individual Education Plan
We provide Supported Community Living Site and Hourly Services for people with disabilities through the HCBS Medicaid Waiver program. This program specifically makes a point of integrating people with disabilities into the community. Therefore, the support takes place in their homes, not at our agency.
Home and Community-Based Services Residential Waiver Program
- Supported Community Living – These services are for any adult or child that meets the admission criteria. Hourly based services can be provided daily. The support staff assists the individual in learning their daily living skills in their home, apartment or shared living arrangement
- Respite – These services are available to any adult or child living at home with their family or guardian. The program is designed to provide short-term relief for the caregivers, such as a night out, shopping, vacations, etc. This service takes place in the individual’s home and may not be used while the parents or guardians are at work.
- Representative Payee Services – This service assists you with managing your money. Staff will help you with check writing, paying your bills, and manage other aspects of your personal income based on what your needs and level of independence with these tasks are.
The Supported Community Living program is set up to be individualized for what that person wants. The program is flexible to meet the goals of the individual in an environment where the individual’s rights are practiced.
Services provided within this service include:
- Training and support in daily living skills
- Training and support in health needs and services
- Training and support in assessing the resources within the community
- Training and support in community living skills
Concerned, Inc. is licensed and accredited in their residential services by the State Department of Human Services.

Supported Employment
Supported Employment is an employment service for people with disabilities who need assistance in finding a job. Over 75,000 people nationwide have found jobs with the help of supported employment since 1985.
Supported employment is a personnel service in which qualified persons with disabilities are matched to jobs and training by employment specialists at no cost to the employer or the person served.
Our specialists survey employer’s personnel needs, match qualified applicants to open job positions and provide training and follow-up consultation.
Supported Employment can benefit your company through cost savings by reducing overtime, turnover and the cost of hiring and training; through increased sales and service by freeing current staff from routine functions; through the use of a new untapped reserve of capable and dependable employees (people with disabilities) and through access to available tax credits that can be used by employers participating in this program.

Mickel’s Restaurant, Hy-Vee, Time for Tots, Concerned, Inc., Elm Crest Retirement Community, Pizza Ranch, Pizza Hut, Kountry Kids Care, Myrtue Memorial Hospital, Burger King, McDonald’s and Harlan Community Schools
Supported Employment, not only serves the employer, but also the employee with disabilities. Some of the benefits to the employee with disabilities include:
- Job seeking skills training which will help the employee to find the right job.
- Employment Specialists will assist the employee in finding the right job by communicating with the employee, prospective employers and the employee’s family (support system).
- Job coaching is available to the employee after a job is secured until the employee is comfortable in performing job duties and no longer feel the need for job coaching support.
- Once job coaching is no longer required, Employment Specialists will continue to make contacts with the employee and employer through follow-up support.
- Employee selects goals for his/her Individual Plan for Employment (IEP). Employment Specialists will support you in achieving your goals.
Selective Placement
For those individuals who do not need the assistance of job coaching, Selective placement provides the following services.
- Job seeking skills such as developing a resume, completing job applications, setting up interviews, accompanying employees to job interviews and assistance providing necessary accommodations.
- Job Development by contacting prospective employers in search of job openings.
Community Based Assessments
The purpose of a Community Based Assessment (CBA) is to “get to know the person” in order to assist them in obtaining a satisfying job in the community. A CBA is used to:
- Identify the appropriate support a person needs to maintain a job
- Determine a person’s preferences, desires and needs
- Eliminate fear, build self-confidence and motivation
- Evaluate different work environments
All Supported Employment Services are accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF).

Adult Day Services is an organized day activity program designed to assist people in becoming more involved in the community. The program also teaches skills to aid in becoming more independent or maintaining the level of independence already achieved by the individual.
